Dear Parent/Guardian:

It’s our “Get Ready for 7th Grade” clinic. Your student will receive a vision screening and hearing screening as well as the opportunity to get caught up on the required and recommended 7th-grade entry immunizations.

If you do not want your student to participate in the vision screening or hearing screening, please notify the school either by writing or by using the “Opt-Out” form provided on the Juab School District webpage.
