Utah Immunization Rule for Students R396-100 and HB 308

In order to attend school, a student must have proof of receiving the following immunizations. Students WILL NOT be permitted to attend school without receiving ALL the required vaccines for their age group or being on schedule to complete an in-progress series.

It is your responsibility to have your child immunized and to provide the school with a medically verified, date- and dose-specific immunization record for all required immunizations he/she has received. This is required for school admission.

Conditional Enrollment and Exclusion:

With the exception of the 7th grade Tdap and Meningococcal, a child may be allowed to attend school “conditionally” if at least one dose of each required immunization has been completed and the child is currently on schedule to receive the remaining immunizations. The remaining immunizations must be completed on schedule for the child to remain in attendance.

Conditional enrollment – HB 308 Section 53A-11-306

Conditional enrollment period is 21 calendar days after the day the school provides written notice to the student’s legally responsible individual, in person or by mail. However, at the end of conditional enrollment period, students who do not comply with the conditional enrollment rule must be prohibited (suspended for non-compliance) from attending school until they provide proper documentation of immunization records to the school.

Immunization Exemption:

A student may be exempt from immunizations for medical, religious, or personal reasons. Legally responsible individuals who have students claiming exemptions for required immunizations must provide new exemption forms routinely when entering: Pre-K and Kindergarten and 7th grade.

Starting July 1, 2018, students claiming an exemption to the required vaccinations must have their legally responsible individual complete an on-line educational module (free of charge), or in-person consultation (fee required) at Juab County Health Department, and provide the completed form to the school. The exemption form must be kept in the student’s cumulative file at the school. Photocopies or faxes are not acceptable. In the event of an outbreak, students who have claimed an exemption or are on conditional enrollment, and have not received the immunization for which there is an outbreak, are to be excluded from school.

Exceptions: If a student changes schools before kindergarten enrollment (PK, daycare), the current exemption form will be valid until the student enters kindergarten, or turns 6 years old, whichever comes first. If a student changes schools after kindergarten, but before 7th grade, the current exemption will be valid until the student turns 12 years old, or enters 7th grade, whichever comes first. The exemption obtained at 12 years of age, or at 7th grade will be valid through 12th grade. Two-year Rule: In addition to the exceptions above, all exemption valid for a minimum of 2 years. For example: If a student obtains an exemption in 6th grade, the student will not be asked to provide a new exemption until 8th grade.

Pre-K

• DTP: 3-4 vaccines (depending on age)
• Polio: 3-4 vaccines (depending on age)
• MMR: 1 vaccine (must be given after the 1st birthday)
• HIB: 3-4 vaccines (depending on age)
• Hepatitis B: 3 vaccines
• Hepatitis A: 2 vaccines
• Varicella: 1 vaccine
• Pneumococcal: 3-4 vaccines (depending on age)

Kindergarten

• 5 DTP (4 doses if 4th dose was given on/after the 4th birthday)
• 4 Polio (3 doses if 3rd dose was given on/after the 4th birthday)
• 2 Measles/Mumps/Rubella (first does must be given on/after the 1st birthday)
• 2 Hepatitis A
• 3 Hepatitis B
• 2 Varicella (history of chickenpox is acceptable, but only if the parent provides the school with a document signed by a healthcare provider as proof of immunity)

Grades 1 – 6

• 5 DTP (4 doses if 4th dose was given on/after the 4th birthday)
• 4 Polio (3 doses if 3rd dose was given on/after the 4th birthday)
• 2 Measles/Mumps/Rubella (first does must be given on/after the 1st birthday)
• 2 Hepatitis A
• 3 Hepatitis B
• 2 Varicella (history of chickenpox is acceptable, but only if the parent provides the school with a document signed by a healthcare provider as proof of immunity)

Grade 7 – 10

• 5 DTP (4 doses if 4th dose was given on/after the 4th birthday)

Grades 11 – 12
